What can I do if the medical records state untruths in terms of the number of prescriptions issued? 4 prescriptions were issued according to the documentation, of which 2 were cancelled by the doctor and did not enter it in the medical records? (I only have an unsigned letter from the doctor provided by the clinic). The clinic refused to correct me because it did not recognize my mailing address - so I wrote the same letter, indicated the "correction of the correspondence address" and demanded confirmation of receipt on the spot. I run out of the drug, it became unavailable in Poland on the way and doctors on the basis of documentation state that they will not write me a prescription, because I still have 2 reepts for a total of 6-7 months according to the documentation). To which office should I go to motivate the doctor to perform activities that he refuses to do (refuses to enter in the documentation / system of the clinic). The visit was private. The Patient Ombudsman does not see a problem and has not indicated to me the appropriate institutions where I can bring an accusation or complaint. The manager of the clinic, of course, knows everything. What to do if the manager does not respond to the letter, answers evasively (i.e. there will be no documentation correcting the number of prescriptions? The name of the substance, the amount and the dose were correct according to the "patient printout". The prescription stopped scanning after 10 days from the moment of its issuance (previously it scanned for which I have proof in the form of a printout from the pharmacist). Currently, "prescription incorrect or canceled" - this is what the same pharmacist sees. Same in another pharmacy. Just do not write to go to the doctor - I will go as soon as I have the correct documentation so as not to get the answer "you still have prescriptions". The doctor does not believe what the pharmacist wrote for the reason that he is not able to confirm that it is an unmistakable prescription. There is no signature and stamp and it is not a document according to 2 doctors on whom I lost over PLN 400. Digitalization is cancer. Previously, I never had this problem because it was impossible to cancel patients' prescriptions without their knowledge - you would have to steal it. Now you leave the doctor practically without knowing what is on the prescription (because many refuse to print and we do not agree to IKP), and if you even get a printout - they can cancel it all. Digitalisation is an action to the detriment of citizens!!!
