Your observations are very important and require immediate consultation with a doctor. Salmex contains salmeterol (a bronchodilator), which may cause side effects such as increased blood pressure, palpitations and headaches in some patients.
Salmeterol can lead to hypokalaemia (a decrease in potassium levels), which, in combination with Co-Valsacor (which contains hydrochlorothiazide – a diuretic that can also flush out potassium), poses a significant risk to the heart.
Attenuation of the effect of a hypertensive drug by steroids or beta-mimetics contained in inhaled drugs such as Salmex is a known clinical interaction and requires a correction of the dosage or a change in the preparation by a specialist. Make sure that you rinse your mouth with water each time you use Salmex to minimise systemic absorption.
In this situation, I encourage you to keep a diary of blood pressure measurements and report to your doctor (pulmonologist or cardiologist). Salmex must not be discontinued on your own, but potassium levels may need to be balanced and hypertension treatment modified.

Salmex may uncommonly cause cardiac arrhythmias, e.g. supraventricular tachycardia, accessory contractions and atrial fibrillation, and a small and transient decrease in serum potassium when used at high therapeutic doses. Therefore, Salmex should be used with caution in patients with severe cardiovascular impairment, including cardiac arrhythmias, diabetes, hyperthyroidism and untreated hypokalaemia, or in patients with a tendency to low serum potassium ion levels.
Headaches are a very common side effect of Salmex.
You should discuss with your doctor the possibility of using other medications if the side effects of your current treatment make it difficult to function on a daily basis. I encourage you to consult a specialist conducting your treatment in this matter.
