Why do I have to take the first dose of Hydroxyzinum at bedtime to treat scarlet fever?

Thank you so much. If I can ask you for an answer, when can I expect the rash to go down? Antibiotic (penicillin) ospen 1.5 million every 8h. Today I started the 4th day, and it is prescribed for 10 days. The rash appeared as standard on the 2-3rd day of treatment. I will be very grateful for your answer and possibly for advice on whether you can additionally support the treatment of rashes? I am currently using Alantan oiling ointment. Thank you
