Pentazocinum 30 mg and Dolcontral (Pethidinum) 100 mg Good morning. For several months it has not been possible to get these drugs in the pharmacy. They have no counterparts with the same compositions. I have been suffering from arthrogryposis since birth. These ampoules save my life in some situations. I am under the constant care of the Pain Clinic. Other drugs are not as effective. Doctors spread their hands. Why did these two drugs disappear from pharmacies ?? Please let me know.

To the best of my knowledge, the withdrawal of these drugs from the manufacturers' offer has been due to the reduction of indications for the use of these drugs. Declining demand may have influenced the business decision to end production. Ultimately, however, we do not have a statement from the manufacturer on this matter. The pharmacokinetics of pethidine (the active ingredient of drugs such as Dolargan or Dolcontral) causes that the analgesic effect of this drug is short, while the neurotoxic effect of its metabolite (norpetidine) may accumulate due to the long persistence of this compound in the body. This is a risk especially when repeated doses. Pethidine in hospital medicine was used in acute colic or postoperative pain, but the possibility of its use in chronic pain was limited by the already mentioned short half-life. Pethidine was also used more widely in gynecology, but due to the risk of exposure of the newborn to this drug, it was withdrawn from use during childbirth. The adverse effects of pethidine were also noticed in seniors. Slower metabolism, reduced kidney function - favored the accumulation of the above-mentioned, adversely acting norpetidine. An increase in the frequency of adverse reactions was also observed. Due to the decreasing number of indications for the use of the drug and the possibility of replacing it with other opioids - it has lost its importance. The drug ceased to be mentioned in the standards of pain treatment, and doctors reached for this preparation less and less. Similarly, in the case of pentazocine - it has an analgesic effect several times weaker than morphine, while it has hallucinogenic, psychotomimetic and dysphoric effects - generally it is about causing side effects in the mental sphere and on the nervous system. This profile of action of the medicine meant that its use was no longer recommended in many guidelines.
