With this question, it would be best to turn to a specialist in the field of psychiatry. To my knowledge, in most cases (especially in the first years of the disease), a patient in a state of mania or hypomania is not able to distinguish hallucinations from reality. He considers this state realistic and has no awareness of the change in his behavior. Nevertheless, during the remission of the disease - that is, when the mania / hypomania calms down, the patient remembers his behavior from the time of mania and can identify them as incorrect, ridiculous, contrary to his previous healthy behavior.
