A decrease in contraceptive protection after 12 hours after not taking the contraceptive pill is predicted only on certain days of the cycle, when the level of hormones in the body is not yet at the appropriate level to protect the patient from pregnancy. As for why the patient is protected during the 4-day placebo period - regular intake of active tablets makes the body for 24 days "work out" hormone levels that are sufficient to protect against pregnancy during the placebo period.
