You cannot switch from Elestar HCT 40+10+12.5 to Egiramlon 10+10 without consulting your doctor, as they are two different medicines - they are not equivalent and cannot be used interchangeably.
Elestar HCT 40 mg/10 mg/12.5 mg contains 40 mg olmesartan + 10 mg amlodipine + 12.5 mg hydrochlorothiazide and Egiramlon 10 mg + 10 mg contains 10 mg amlodipine + 10 mg ramipril. As you can see, the only common point for both preparations is 10 mg of amlodipine. In addition, the composition of the drugs differs significantly - Elestar HCT is a three-component drug, Egiramlon contains only two ingredients.
If you have any concerns about your treatment with Elestar HCT, it would be best to talk to a doctor who has full access to your medical records. On the other hand, you can make an appointment for a teleconsultation at GdziePoLek:

I am also attaching additional information about the medicines you are interested in. You can book prescription preparations at GdziePoLek, but you have to pick them up at the store after presenting a prescription:
