The recommended starting dose is 5 or 10 mg, orally, once a day, both in patients who have not been previously treated with other statins and in patients treated with other HMG-CoA reductase inhibitors. When determining the starting dose, the patient's cholesterol levels and risk of future cardiovascular disease as well as the risk of side effects should be taken into account.
Due to the increased incidence of adverse reactions at 40 mg compared to lower doses, dose escalation to a maximum dose of 40 mg should only be considered in patients with severe hypercholesterolaemia who are at high risk of cardiovascular disease (particularly patients with familial hypercholesterolaemia) who have not achieved the expected therapeutic goal at the 20 mg dose.
