Hello, the cardiologist prescribed me Triplixam 10 mg + 2.5mg + 5 mg after 2 weeks of using 1x1 I did not notice much improvement. The pressure varies up to 195/96. I joined Spironol 25mg after which there are also no effects. What to do? The pressure does not drop.

There is an algorithm for the management of patients who do not respond to treatment and with suspected refractory hypertension. Adding a diuretic such as Spironol is a common next step, and in the absence of improvement, the doctor has the option to change the doses of the drugs, as well as add another drug as part of the intensification of treatment. Patient-dependent and often removable causes of resistance to hypertension treatment are non-compliance with medical recommendations, lack of lifestyle changes (consumption of excessive amounts of salt and alcohol, too much body weight), as well as taking drugs without consulting a doctor that can raise blood pressure. Sometimes it is helpful to carry out tests to detect secondary hypertension, i.e. diseases that, if left untreated, cause a sustained increase in pressure. Truly resistant hypertension is rather rare, but in such cases, surgical treatment may be the solution. I enclose additional information about the medicines you use.
