For @Karolina: In such cases, drugs that inhibit the secretion of hydrochloric acid (PPIs) or antacids (m.in. salts of aluminum, calcium, magnesium) and drugs protecting the mucous membrane (alginic acid, hyaluronic acid in combination with chondroitin sulphate) or prokinetic drugs are used. IPP or antacids can be purchased at a pharmacy without a prescription and on-line with home delivery, e.g. https://www.gdziepolek.pl/produkty/19739/controloc-control-tabletki/apteki https://www.gdziepolek.pl/produkty/11808/bioprazol-bio-max-kapsulki/apteki https://www.gdziepolek.pl/produkty/3972/alugastrin-zawiesina/apteki Prokinetic drugs are available on prescription: e.g. https://www.gdziepolek.pl/produkty/64348/prokit-tabletki-powlekane/apteki If you need to consult doctor, here you can use the teleadvice: https://www.gdziepolek.pl/telekonsultacja

Why can't I buy a drug called Jorveza in pharmacies? Is it available in any pharmacies in Poland?

Are there medicines for allergic esophagitis available in pharmacies? By this question, I do not mean eosinophilic esophagitis. If so, under what name? How long is allergic esophagitis treated?

For @Agnieszka: The drug is available and registered in Poland, so it is not possible to import it in the target import mode. Only drugs that are not approved for marketing in Poland and are necessary to save the patient's life or health can be imported from the target import. Most often, medicines are imported from another country of the European Union, but there are no obstacles to importing medicines even from another continent by way of target import. Unfortunately, Jorveza is not one of these types of medicines. The current availability of the drug can be checked here: https://www.gdziepolek.pl/produkty/117538/jorveza-tabletki-ulegajace-rozpadowi-w-jamie-ustnej/apteki/w-polsce

I understand. And is it then possible to import Jorvez's drug from abroad? If so, what is the procedure? And what is the price of the drug portfolio in terms of PLN?

To @Agnieszka: as I have already written, Budesonide LEK-AM is not a preparation for esophagitis, but a drug for inhalation in respiratory diseases. The use of budesonide from an aerosol from a nebulizer or in the form of viscous suspensions prepared from a nebulization preparation or capsules administered orally is an off-label treatment. The doctor decides, taking full responsibility for such treatment and indicating to the patient the exact method of using the drug beyond what the manufacturer allows. Therefore, you should ask your question to the gastrologist who is leading you.

Thank you. So, can Budesonide LEk-Am be used for EoE?

For @Agnieszka: Jorveza is an orodispersible tablet that contains 0.5 mg or 1 mg of budesonide. Jorveza has indications for use in adults for the treatment of eosinophilic esophagitis (EoE). The preparation has no registered equivalent. Budesonide LEK-AM is an inhalation powder, hard capsules, containing 0.2 mg or 0.4 mg of budesonide. Budesonide LEK-AM is used to reduce inflammatory changes in the lungs and helps maintain airway patency. Do not swallow Budesonide LEK-AM capsules – they should be inhaled using the inhaler included in the pack.

Thank you. What is the difference between Jorveza and Budesonide Lek-Am? Does Jorveza have substitutes? If so, under what name?

As I understand it, you mean eosinophilic esophagitis. If so, then dietary and pharmacological treatment is used, and in the case of narrowing of the esophagus - endoscopic expansion of the esophagus. Available pharmacological therapy options include corticosteroids administered topically into the esophagus (fluticasone or budesonide) and rarely generally (prednisone) and proton pump inhibitors (omeprazole or equivalent drug). Proton pump inhibitors are widely available on the Polish market. As for glucocorticosteroids, budesonide is mentioned in the form of tablets that break down in the oral cavity (Jorveza - according to data from July 2022, this drug has not yet appeared on the market in Poland, it is classified as orphans, it has indications for the treatment of eosinophilic esophagitis). There is information that gastrologists are using budesonide products from inhaled drugs appropriately prepared for oral administration as an off-label therapy: https://www.gdziepolek.pl/produkty/117538/jorveza-tabletki-ulegajace-rozpadowi-w-jamie-ustnej/apteki/w-polsce https://www.gdziepolek.pl/artykuly/leczenie-off-label-poza-wskazaniami New biologics used in other allergic diseases dupilumab, cendakimab and benralizumab have shown promise in the treatment of eosinophilic esophagitis, but they do not have a strong enough recommendation at the moment - it is difficult for me to answer whether gastrologists therefore propose them in therapy.
