Have there been any such side effects during the use of Metforal itself? If not, it looks like the side effects are directly related to the change in medication.
Synjardy is a medicine used to treat diabetes that contains two active substances: empagliflozin and metformin. Empagliflozin is a sodium-glucose cotransporter type 2 (SGLT-2) inhibitor that acts at the kidney level to reduce the absorption of glucose from urine into the blood, which can lead to loss of calories through urine.
Empagliflozin may cause slight weight loss in some patients. This is a side effect related to its effect on the body's water and electrolyte balance and the loss of calories along with glucose excreted in the urine. Nevertheless, weight loss is not always guaranteed, and its intensity may vary from patient to patient. Losing almost 20 kg is a significant weight loss, which I would advise you to consult with your doctor. On the other hand, spots on the legs look like diabetic dermatopathy - a complication of diabetes resulting from unbalanced blood sugar levels. Diabetic dermatopathy is manifested by the formation of spots - not very large, which can increase in size over time. It is characteristic that initially they are light brown and dark brown, with time they take on a pink or red color.
It may turn out that switching to Synjardy was not the right change and you should discuss changing the therapy to a different one with your specialist.
