This combination can be used, however, requires adjustment of the insulin dose. Patients treated with semaglutide in combination with a sulphonylurea or insulin may be at increased risk of hypoglycaemia. The risk of hypoglycaemia can be reduced by lowering the dose of a sulphonylurea or insulin when initiating semaglutide therapy. Semaglutide should not be used in patients with type 1 diabetes or for the treatment of diabetic ketoacidosis. Semaglutide is not a substitute for insulin. Diabetic ketoacidosis has been reported in insulin-dependent patients as a result of abrupt interruption or dose reduction of insulin after initiation of GLP-1 receptor agonist therapy (e.g. Ozempic).
