Juzimette (containing metformin and sitagliptin) usually does not directly cause chest pressure or sudden pressure spikes, so these symptoms should not be considered 'normal'. The gastrointestinal discomfort described may be gastrointestinal (e.g. severe heartburn or bloating, which often occurs at the beginning of metformin and mimics tightness in the chest), but rarer, more serious reactions such as hypersensitivity or cardiovascular effects cannot be excluded.
I encourage you to measure your blood pressure and heart rate regularly and contact your doctor to rule out other cardiac causes or side effects of the drug. Do not discontinue the medication on your own without consultation.
