To get vaccinated against shingles in a pharmacy, you need to find a pharmacy near your place of residence that provides vaccination services.
You can search for a vaccination pharmacy here:

In a pharmacy, a pharmacist will qualify for vaccination free of charge, issue a pharmaceutical prescription for the Shingrix vaccine and vaccinate – all during one visit to the pharmacy.
You only cover the cost of the vaccine. And here is an important point:
People over 65 years of age can receive the vaccine free of charge as part of the "Medicines 65+" program. However, in order to benefit from the reimbursement, it is necessary to confirm from a doctor that the patient belongs to the group of increased risk of developing shingles (m.in. people with heart, kidney, liver, diabetes or chronic lung diseases). Otherwise, the cost of the vaccine is PLN 751.16 for 1 dose.
Reimbursement is also available to people over 18 years of age with chronic diseases belonging to risk groups. In this case, the price of the vaccine is PLN 375.58 for 1 dose. Here, too, a certificate from a doctor confirming a chronic disease is required. Without reimbursement, the cost of the vaccine is PLN 751.16 for 1 dose.
The full list of indications for reimbursement can be found here:

Instead of a certificate, you can also immediately ask the doctor for a discounted prescription, which should then be presented to the pharmacist. The pharmacist will dispense the vaccine and can immediately administer it to the pharmacy (the vaccination service will be free of charge).
To ensure protection, it is necessary to take 2 doses of the vaccine at intervals of about 2 months.
