I mistakenly took 1 pill on the first day, instead of two. Today is the third day, can I take the 2nd pill today? Or take the medication one day longer?

Take the missed dose of Azithromycin as soon as possible and then continue treatment as directed by your doctor.

In the case of azithromycin, a dose schedule of 1 g (2 tablets of 500 mg) on the first day and then 500 mg (1 tablet) for the following days is often used. If you only took 1 tablet instead of 2 on the first day, you can take two tablets today, but do not increase the total dose above the recommended dose.

If in doubt and need to prolong the therapy, it is worth consulting a doctor, as the duration of treatment and doses may vary depending on the indications and type of infection. Azithromycin can also be used for 5 or 6 days if your doctor so recommends. It is important not to exceed the recommended cumulative dose and avoid the risk of overdose.
