Gentamicin is an antibiotic that starts working within a few hours after administration, but the effects of treatment may not be visible until 48-72 hours. If you do not see any improvement after the second injection, it is worth reporting it to your doctor, because chest pain can have various causes, requiring additional diagnostics or change of therapy. Please remember to take the recommended series of injections in full for the treatment to be effective.
