Yes, Zivafert is only available with a prescription. A prescription issued in a country other than Poland (e.g. issued in Ukraine) should contain the following information:
- patient's name(s), patient's address;
- a commonly used (international) name or trade name, form, strength, quantity;
- the date the prescription was issued;
- details of the person issuing the prescription in the form of an imprint or stamp and the handwritten signature of the person issuing the prescription.
