Please note that the final decision rests with the attending physician. It is difficult to give unambiguous information on the basis of such a short description.

According to the information contained in the current reimbursement list, the indication for reimbursement of Forxiga is, m.in:
Chronic heart failure in adult patients with reduced left ventricular ejection fraction (LVEF≤40%) and persistent symptoms of NYHA class II-IV disease despite the use of ACEi-based therapy (or ARB/ARNi) and beta-blockers and, if indicated, mineralocorticoid receptor antagonists.

So, if you meet the above conditions, you are entitled to Forxiga at a discount.
