Hello, the last 5 days I have been taking Trioxal (itraconazolum), I now have throat infections and I need to start taking the antibiotic Sumamed (azithromycin). I know that these two drugs interact with each other seriously. I read on the internet that the half-life of itrax is from 24 to over 40 hours. So 40 hours after the last dose of trioxal, can I safely start taking Sumamed? Thank you very much in advance for your reply.

The half-life of 24-40 hours is not the time after which the drug will be completely excreted from the body. To calculate after what time the drug will be completely excreted from the body, several other components are needed. However, in the case of Trioxal, excretion from the body takes a long time - on average from 7 to 14 days. The concentration of the drug in the blood decreases gradually. Half-life: The half-life is a pharmacokinetic parameter that means the time it takes for a drug substance (API) in the blood to drop to half its initial value. According to the manufacturer of Trioxal: "After discontinuation of the drug, within 7 to 14 days, the plasma concentration of itraconazole decreases to almost undetectable values, depending on the dose and duration of treatment." It is true that the combination of Trioxal and Sumamed can lead to arrhythmias. However - not every patient has these symptoms. Therefore, if you have already finished Trioxal treatment and it is necessary to take Sumamed - you can take an antibiotic according to your doctor's recommendation. It is worth starting antibiotic therapy as early as possible to stop the development of infection.
