If I didn't manage to put on a tampon, could I have been left with a cotton wool attack in my vagina?

A good quality tampon should not leave fibers or "cotton wool" in the vagina. Since you didn't actually put it in, but only "touched the entrance to the vagina" for several seconds, I don't think anything could "stay there". The vagina also has the ability to self-cleanse, so even if a particle separates, this should not be a problem.
