What hypoglycemic drugs do you need to use to have a discount?

Witam.Mam question about the reimbursement of the drug joridance. You have to meet certain conditions, one condition is that you need to take two hypoglycemic drugs. Question. Is there two drugs and joridance, or one other drug and joridance? Nowhere can I find information has this topic.

I suppose you are referring to the drug Jardiance, not "joridance". Current reimbursement indications for Jardiance are: Type 2 diabetes, in patients before insulin inclusion, treated with at least two oral hypoglycemic drugs for at least 6 months, with HbA1c ≥ 8% and very high cardiovascular risk understood as: 1) confirmed cardiovascular disease, or 2) damage to other organs manifested by: proteinuria or left ventricular hypertrophy or retinopathy, or 3) the presence of 3 or more of the main risk factors listed below: -age ≥ 55 years for men, ≥60 years for women, -dyslipidemia, -hypertension, -smoking, -obesity; Chronic heart failure in adult patients with reduced left ventricular ejection fraction (LVEF≤40%) and persistent symptoms of disease in NYHA class II-IV despite the use of ACEi(or ARB/ARNi) therapy and beta-blockers and if indicated by mineralocorticoid receptor antagonists As I understand you are interested in the fragment "treated with at least two oral hypoglycemic agents for at least 6 months". This should be understood as meaning that the patient must attempt to regulate the level of sugar levels with at least two hypoglycaemic drugs other than Jordiance (e.g. metformin plus a sulphonylurea) and only if this is ineffective or insufficiently effective, the doctor may consider prescribing reimbursed therapy with Jardiance. However, if you already use Jardiance by paying for it for 100%, it may be difficult to show indications for issuing discount prescriptions - how to assess whether another therapy (e.g. with the aforementioned two-drug combination) would not be effective enough? Since the doctor decides about issuing a discount prescription, I suggest you talk to him directly about your individual case. All information should be in your medical records?
