Should I really not give such a strong drug to a child, or maybe in other doses?

Hello. Today the daughter was prescribed Sumamed 500 3 tablets. The daughter is 6.5 years old, weighs 37 kg. In the pharmacy, the pharmacist did not want to dispense the drug, but after consulting another doctor, (because the family doctor has already finished work today) when the doctor stated that the doctor's signature on the prescription obliges, the drug was issued. Should I really not give such a strong drug to the child, or maybe in other doses? Can break the tablet and instead of 3 days give 6 days in half?

The dosage of Sumamed depends on the disease for which the medicine was prescribed - to assess whether the dose of the drug is correct, you should know the detailed diagnosis of the doctor, e.g. a record in the patient's card, so in deciding whether the prescribed dose is correct, the right course of action is to contact the doctor who has examined the child, and if not - another who has access to the child's records. The pharmacist therefore did the right thing. Justifying my position, I will give the following examples: 1. In upper and lower respiratory tract infections, otitis media and skin and soft tissue infections (except erythema migrans), the total dose of azithromycin - the active substance of Sumamed - is 30 mg / kg, or 10 mg / kg once a day for 3 days. In this case, a child weighing 37 kg can take a daily dose of 370 mg. 2. For the treatment of erythema migrans, the total dose shall be 60 mg/kg administered as follows: 20 mg/kg on the first day, then 10 mg/kg once daily from day 2 to day 5. In the case of a child weighing 37 kg, it is 740 mg of azithromycin on the first day and 370 mg from the 2nd to the 5th day of treatment. 3. There are other diseases in which the daily dose of azithromycin in children is 12 mg / kg, i.e. in the case of a child weighing 37 kg - 444 mg per day. As you can see - in order to be able to objectively assess the dose that your child should take, a detailed knowledge of the state of health is necessary. If the doctor considers that a 250 mg or 500 mg tablet/capsule is too high a dose for the child, he decides to save the drug in the form of a suspension, where you can very precisely measure the amount of medicine needed in ml. You should not modify the dosage in any way, e.g. break the pills, because we have no grounds to do so - we do not know the diagnosis. To fully dispel doubts, you can call the doctor who ordered the medicine and tell you directly about your concerns. The doctor will certainly help to dispel them, and if you feel safer, you can decide to save the drug in the form of syrup.
