What should I do if two doctors recommend different medications?
Hello, I am 21 years old, I got my first menstruation at the age of 16 - the next one did not appear. In 2018, I used lutein prescribed by a gynecologist – it did not work. In 2019, I took Femoston for half a year (when taking pills, the period occurred, but after discontinuation it did not appear.) And recently: on August 10, 2021, I unexpectedly got bleeding. My current results (done on day 2 of bleeding): lh 0.42 ; estradiol 25 ; fsh 1: 34 ; tsh 1,980 ; ft4 12.35 . From bone tests I get osteopenia. I will add that since the age of 12 I have been struggling with eating disorders, in the last year I managed to get out of anorexia. I've been to two doctors in the last week. The problem is that they presented me with completely different ways of treatment. The first doctor said that I had to take Femoston for 8 months because the results were very poor and there was no other option. The second doctor said that I do not have menopause and therefore Femoston is not needed for anything - he ordered to take sylvie tablets for 3 months. Question: what to do? Please help

Zaburzenia cyklu związane były w Pani przypadku najprawdopodobniej z anoreksją. Wystąpienie fizjologicznej miesiączki (nie pobudzanej lekami) należy wiązać z sukcesem w walce z chorobą, stabilizacją wagi i odtworzeniem tkanki tłuszczowej. Osteopenia jest skutkiem zbyt małego stężenia estrogenu, który odpowiada za prawidłową strukturę kości. Przy wtórnym (wywołanym w Pani przypadku najprawdopodobniej zbyt niską masą ciała) braku miesiączki, jednym ze sposobów terapii jest włączenie substytucji estrogenowo-progestagenowej, czyli dostarczanie hormonów z zewnątrz.
Z farmaceutycznego punktu widzenia mogę poinformować Panią, że oba leki - zarówno Femoston, jak i Sylvie - zawierają hormony płciowe estrogen i progestagen. Ze względu na skład i dawki mają one inne wskazania do regularnego stosowania, natomiast nie ma przeszkód, aby lekarz wykorzystał ich działanie w innych przypadkach, jeśli będzie upatrywał w tym korzyść dla pacjentki.
Niestety recenzowanie zaleceń lekarzy i rozstrzyganie co do słuszności lub nie ich postępowania oraz interpretowanie wyników Pani badań, nie jest możliwe na forum internetowym. Jeśli ma Pani wątpliwość, jak postąpić, to proszę skonsultować się z innym specjalistą.
