The dosage of clarithromycin in children is based on body weight (usually 15 mg/kg/day in 2 doses), with the prolonged-acting form generally intended for adults and adolescents and only for use in children if specifically advised by their doctor.

At 61 kg, doses close to adults may be appropriate, but it is not safe to administer 500 mg alone without a doctor's recommendation. Please consult your doctor - the choice of dose also depends on the indication, the duration of treatment and any comorbidities. If the drug has already caused side effects, the next dose should be withheld until consultation.
