What should I do if my heart pain does not go away with Colchicine for pericarditis?

I have pericarditis for 3 weeks . They gave me Colchicine 2 times a day for 28 days. For now I take a week of time and recently the heart pain began to deteriorate? Why instead of it should be better, and why does it intensify after 11 p.m. every day? I'm very worried about that.

Colchicine is recommended for the treatment of acute gout attacks. It can also be used to prevent acute attacks of gout when starting treatment with other drugs, such as allopurinol, probenecid, for example. The use of the drug in the treatment of pericarditis is the so-called "off-label" recommendation, i.e. outside the registered indications. The decision to use an off-label drug is made by the doctor on the basis of, m.in of his clinical experience. Available clinical studies indicate the effectiveness of colchicine in the treatment of pericarditis, as well as the prevention of its recurrence. We wrote more about "off-label" treatment in the article: https://www.gdziepolek.pl/artykuly/leczenie-off-label-poza-wskazaniami If the heart pain worsens and does not go away, I encourage you to consult your doctor again. You can also use medical teleadvice via, for example, the portal Gdziepolek.pl: https://www.gdziepolek.pl/telekonsultacja/z-kardiologiem So far, no heart pain has been indicated after the use of colchicine preparations. Side effects of the drug include, m.in, inflammation and nerve damage, gastrointestinal bleeding, liver damage, alopecia, menstrual disorders. If you suspect that the use of the drug worsens the condition of the heart, I encourage you to consult, as well as to report the suspicion of side effects not described in the leaflet. Reporting adverse reactions allows you to increase control over medicines available on the market. More about how and why to report side effects, we wrote in the article: https://www.gdziepolek.pl/artykuly/jak-i-dlaczego-zglaszac-dzialania-niepozadane I enclose a link to check the availability of Colchicine and make a reservation at the selected pharmacy: https://www.gdziepolek.pl/produkty/92267/colchicine-genoptim-tabletki/apteki
