What does it mean that Abilify is a partial dopamine agonist?

How is it possible that Abilify is a dopamine agonist (where there is too little) and an antagonist (where there is too much dopamine)? THE DOCTOR DID NOT ANSWER ME WHAT HE MEANT. other dopamine agonists lowered my anxiety and added self-esteem, but Abilify boosts my fears and feelings of worthlessness and worry (not for the first time)

A unique feature of partial agonists is that they have a differentiated effect depending on the environment: they block receptors in the presence of agonists with higher internal activity, but they themselves act as agonists. (Kiss B, Horváth A, Némethy Z, Schmidt E, Laszlovszky I, Bugovics G et al. Cariprazine (RGH-188), a dopamine D(3) receptor-preferring, D(3)/D(2) dopamine receptor antagonist- partial agonist antipsychotic candidate: In vitro and neurochemical profile. J. Pharmacol. Exp. Cariprazine, another partial agonist D2 and D3, has also sometimes caused a side effect in clinical trials in the form of anxiety; however... He won the ranking of drugs reducing the negative symptoms of schizophrenia (avolicia, apathy, social withdrawal) and is also supposedly good for cognitive symptoms (lack of concentration, memory disorders).

There are many types of proteins in the nervous system called receptors that, when combined with a medicine, react in a certain way. Aripiprazole (the active substance in Abilify) has a multidirectional mechanism of action, namely: it binds to certain receptors in the nervous system, causing nerve conduction to increase or inhibit. When a drug binds to a protein to inhibit its activity, we say that it is its antagonist, and if the drug activates a given receptor, we call it an agonist. Aripiprazole due to its chemical structure is the so-called partial agonist of dopamine receptors. That is, it stimulates them, but not to the same extent as a full agonist. In addition, aripiprazole affects other receptors, m.in serotonin (some inhibitory, others stimulant). Abilify therefore has a rather unique mechanism of action compared to other antipsychotics. Side effects may occur at the beginning of treatment (as with most drugs in this group), it is recommended to increase the dose gradually. The full effect of the drug (especially the anxiolytic effect) develops at higher doses. Please remember, however, that dose adjustment is the competence of the doctor, and as a pharmacist I do not have the authority to assess the validity of therapy on the basis of your brief description.
