Pain at the injection site is an uncommon side effect of Cetraxal. The described problem may result from both hypersensitivity to the drug itself and the presence of inflammation, which generates irritation and a pain reaction when the drops are inserted into the ear. If the problem is so severe that it hinders normal functioning, you should consult an ENT specialist. Perhaps another drug will be better tolerated.
