Alopecia is one of the side effects of tamoxifen, however, 5 years from the start of use is quite a long time to associate it as a side effect of therapy. Hair can also fall out for other reasons - stress, other medications, thyroid disease, anemia - it is worth doing research and see if there is another suspicious source of this problem.
