My son has been taking Modigraf 1mg for a few years, now the doctors want to switch to Advagraf 1mg, I'm skeptical about it because every change carries a risk, how are these drugs different? Is the main substance released in the same way, or do they differ in the composition of the excipients? Modigraf works well for him, is it worth changing? Please help me to understand this.

Both medicines, Modigraf and Advagraf, contain the active substance tacrolimus. Tacrolimus is an immunosuppressive medicine used to prevent rejection of a transplanted organ.
The main difference between Modigraf and Advagraf is in the form of application. Modigraf are sachets. In order to achieve maximum absorption, the granules should be taken on an empty stomach or at least 1 hour before a meal or 2-3 hours after a meal. The drug is administered orally 2 times a day, in the morning and in the evening.
Whereas Advagraf contains extended-release microcapsules, which means that the active ingredient is released gradually over a longer period of time. The drug is used once a day, in the morning. To ensure maximum absorption, the capsules should be taken on an empty stomach, or at least 1 hour before or 2-3 hours after a meal.
Advagraf is often chosen by doctors due to its more convenient administration schedule – one capsule once a day is often sufficient. In practice, both drugs have similar immunosuppressive efficacy.
