In general, there is no contraindication to the use of a particular diet due to the metformin taken. In contrast, a ketogenic diet in the full sense of the word is associated with a state of ketosis, and ketosis is one of the risk factors for lactic acidosis (a rare but dangerous side effect of metformin). Lactic acidosis is manifested by shortness of breath, abdominal pain, muscle spasms. If suspicious symptoms occur, the patient should seek immediate medical attention. Many patients recognize that they are on a ketogenic diet, but in fact, after counting macronutrients, it turns out that it is more of a low carb diet, i.e. with a low proportion of carbohydrates, and the patient himself does not lead to a state of ketosis. The ketogenic diet is a very difficult and restrictive diet, so in my opinion it should be introduced and monitored by a specialist if there is a need for it. This is not a question that can be answered unequivocally: you can or you cannot. It is more about analyzing the specific pros and cons, i.e. the real need for a ketogenic diet, determining whether the patient is actually in ketosis, whether combining these two elements is key to achieving a given therapeutic goal.
