The reimbursement indications are twofold and in each of the two cases quite complex. In general, the drug can be reimbursed in patients with type 2 diabetes or in patients with chronic heart failure, however, it is necessary to meet the following additional criteria: Type 2 diabetes mellitus in patients treated with at least two hypoglycemic drugs, with HbA1c ≥ 7.5% and very high cardiovascular risk understood as: 1) confirmed cardiovascular disease, or 2) damage to other organs manifested by: proteinuria or left ventricular hypertrophy or retinopathy, or 3)the presence of 3 or more of the main risk factors listed below: -age ≥ 55 years for men, ≥60 years for women, -dyslipidaemia, -hypertension, -smoking, -obesity Chronic heart failure in adult patients with reduced left ventricular ejection fraction (LVEF≤40%) and persistent symptoms of disease in NYHA class II-IV despite the use of therapy based on ACEi (or ARB / ARNi) and drugs from the group of beta-blockers and if indicated mineralocorticoid receptor antagonists I enclose additional information about the drug Jardiance and our studies that may be of interest: https://www.gdziepolek.pl/produkty/38817/jardiance-tabletki-powlekane/apteki https://www.gdziepolek.pl/artykuly/ile-kosztuje-zycie-z-cukrzyca https://www.gdziepolek.pl/artykuly/kiedy-lek-nalezy-sie-na-znizke
